Nurse - Supervisor, Care Coordination and Wellness

The Supervisor, Care Coordination and Wellness supports and supervises the coordination of care transition and wellness front line functions, including the RN Advice Nurses, wellness coaching, patient outreach, data extraction, program initiatives, and readmission management team members.
The Supervisor will be providing daily oversight to nursing, wellness and patient outreach services as part of our health care team who provides triage advice services via telephone, VIDEO CONFERENCING, or other means by utilizing the nursing process, input from physicians, and the approved telephone nursing guidelines and protocols, to established patients. 
The ideal candidate for this role is a nurse who has demonstrated leadership experience in a supervisory role and practical experience as an RN in an ambulatory, customer service business like a physician's office setting. A strong understanding of disease management programs and standard best practices related to the transition of care is essential to your success.
Minimum Requirements: 1. AA or BA DEGREE in communication, business or healthcare administration, 2. Graduate of an accredited school of nursing required 3. Be able to obtain and maintain a current RN license issued by the State of California Board of Registered Nurses 4. Wellness Coaching Certification or training and completion within first six months of hire.
Experience/Knowledge/Skills:
1. Minimum three to five years of practical nursing experience
2. Demonstrated knowledge of the following:
a. Scope of practice, for RN's in the State of California
b. Understanding of managed care concepts strongly recommended
c. Medical symptomology and what constitutes urgent/emergent care,  
d. Medical terminology and abbreviations
e. Universal precautions
f. Clinical protocols for vital signs
g. Normal values for vital signs (BP, Pulse, Respirations, Temps)
h. Medical positioning for patients
 i. Scheduling practices/referrals for physician offices preferred
j. Effective teamwork practices between front and back office staff
k. Professional knowledge of clinical nursing protocols with directly transferable emphasis in the areas of disease processes, emergencies, health sciences and pharmacology
l. Demonstrated and proficient skills required in all steps of the nursing process.
